No. A web designer or SEO provider cannot honestly guarantee a permanent first-place organic Google ranking. Google does not sell or promise organic positions, its results change continuously, and even pages that meet all technical requirements are not guaranteed to be indexed or served for a particular query.
That does not mean SEO is random.
It means the provider controls the work, not the outcome.
A good provider can improve the conditions.
They cannot own Google's results page.
Google itself says there are no secrets that guarantee first place
Google's current SEO Starter Guide explicitly says there are no secrets that automatically rank a site first.
Its Search documentation also says:
meeting technical requirements does not guarantee indexing;
positions are not static;
search results change as the web and user expectations change.
That should settle the core question.
If Google itself will not guarantee your position, a web designer cannot reasonably guarantee it on Google's behalf.
Why rankings change
Search is competitive.
Other websites improve.
New businesses launch.
Google changes systems.
User intent changes.
Location changes results.
Device and language can affect results.
The query itself matters.
You may rank strongly for:

There is no single universal position for "the website".

I am much more comfortable promising the work I can control than a ranking I cannot. A client should be able to hold a web designer accountable for technical setup, useful pages, local relevance, internal links, performance and measurement. That is real work even though nobody can guarantee where Google will place the site next Tuesday.
What can a web designer control?
A lot.
Technical accessibility.
Site structure.
Page titles.
Content quality.
Internal links.
Mobile experience.
Performance.
Structured data where appropriate.
Local business information.
Redirects.
Canonical URLs.
Sitemaps.
These are meaningful deliverables.
A designer can also help create pages that better match real search intent. That sits inside the wider work of getting a business website onto Google.
That is legitimate SEO work.
What can they not control?
Google's algorithm.
Competitors.
The rest of the web.
Search demand.
Reviews people leave elsewhere.
Every future backlink.
User behaviour.
Market changes.
That is why the outcome has uncertainty.
Professional work is often about improving probability, not claiming control over systems you do not own.
"Guaranteed first page" can also be misleading
Sometimes the provider targets a phrase nobody searches.
For example:

Congratulations.
You are first.
No customers care.
A ranking guarantee is only commercially meaningful if:
the query has real intent;
the audience is relevant;
the traffic converts.
Position without business value is vanity.
Paid ads are different
Google Ads can buy visibility in paid placements.
That is not an organic ranking guarantee.
If a proposal says:
Guaranteed first page on Google
ask:
Do you mean paid ads?
Organic search?
Google Maps?
A branded search for my own name?
These are completely different.
Local results are especially dynamic
A local search can depend on distance as well as relevance and prominence.
Someone standing in Longford town may see different businesses from someone twenty kilometres away.
So "number one local ranking" needs a specific context.
A screenshot from one phone at one moment is not a universal guarantee.
What should an honest SEO promise sound like?
Something like:
I will make sure the site has sound technical SEO foundations, create pages around relevant customer searches, set up measurement, and improve the site's ability to compete. Rankings and indexing are not guaranteed.
That is less exciting.
It is much more credible.
What should ongoing SEO deliver?
If a business pays monthly, expect actual work.
Content.
Technical improvements.
Search analysis.
Local optimisation.
Link/PR work where appropriate.
Conversion analysis.
Reporting.
Strategy.
The provider should be able to explain what changes each month.
"Maintaining your number-one ranking" is not enough if no meaningful work is happening.
Search Console gives you evidence
Google Search Console can show:
queries;
impressions;
clicks;
positions;
pages;
indexing issues.
That lets you evaluate progress with real data.
The average position metric itself still needs interpretation.
But it is much better than a monthly PDF saying:
SEO health: 93%.

Can someone guarantee their own actions?
Yes.
A developer can guarantee:
the sitemap will be added;
the site will be mobile responsive;
the canonical tags will be implemented;
the agreed pages will be built;
Search Console will be configured;
the content brief will be delivered.
Those are controllable.
This is the right place for guarantees.
Beware "secret relationship with Google"
Google does not provide organic-rank privileges to agencies because they are "Google partners".
Google Partner status relates to Google Ads, not preferential organic rankings.
If somebody implies they can call Google and move your organic position, be very cautious.

Replace the question “Can you get me number one?” with “What exactly will you do, how will we measure it, and what would success look like commercially?” That moves the conversation from a promise about an algorithm to work and evidence you can actually inspect.
The bottom line
Do not hire a web designer because they guarantee first place on Google.
Hire them because they can explain:
what search work they will do;
why it is relevant;
how it will be measured;
what outcomes are realistic.
SEO can create major business value.
It does not need impossible promises to justify itself.
